I want to ask that does this therapy more effective to eradicate h pylori.. amox ig three times a day ,bismuth 120mg three times a day , metronidazole 200mg three times a day and esomeprazole 40mg 2 times a day... my main voncern is on metronidazole i have used it before so it might not work ..furazolidone is not widely spread in my country so i might not able to get it ..so is there other antibiotic that i can use instead of metronidazole or i can just use metronidazole since i heard you saying that metronidazole resistance can be overcomed when using another strong antibiotic

High dose PPI + High dose Amox + Bismuth may already give you a cure rate of 80%.
Metronidazole resistance has been shown to be able to be overcome when used with Tetracycline. Since The H. pylori will not become resistant to Amox and Bismuth, I think you can give this quadruple (PPI + Amox + Bismuth + Metro) a try.
If it failed, you can always repeat PPI + Amox + Bismuth + one other antibiotic.

very good question. In theory, Amox and Tetracycline should counter each other. But in practical they dont. I am not sure why.
So, Yes, tetracycline can be your next choice if your current therapy failed.

You can gradually stop your PPI.
switch to one tablet a day or a tablet every two days.
